What you need to do
Report the death as soon as possible so that we can quickly get in touch with the surviving dependants and assist them in claiming benefits.
Please complete the “Notification of death” form and upload it to the Connect online portal. You can also send us the following documents if you have them:
Copy of the official death certificate
Copy of the family certificate
Any applicable daily allowance statements for calculating exemptions from contributions in cases of inability to work due to illness or accident
The obligation to pay contributions ends in the event of death at the end of the month in which the death occurred.
